The Clinic offers several types of care for our new and established patients such as primary care, women’s health, Take Charge program, DOT physicals, school/sports physicals, pediatrics, and immunizations. We also have same-day appointments available. We file all insurances including Medicare and Medicaid. We also have an income-based program for uninsured patients.

To apply for the income-based program, one of the following proof of income must be presented at the time of check-in:

• Current paycheck stub
• Social Security Statement
• Letter from DHS with the financial services you receive
  

If no proof of income is available, we will accept a notarized letter from the person/place that is assisting you with your financial care. New patients need to arrive 30 minutes prior to the scheduled appointment time. Established patients need to arrive 15-20 minutes prior to the scheduled appointment time.

The Clinic serves more than 28,000 unique patients in 10 Oklahoma counties, with primary locations in Ada, Konawa, Seminole, and Stratford.
