The medical clinic at COFMC offers several types of care for our new and established patients such as primary care, women’s health, Take Charge program, DOT physicals, school/sports physicals, pediatrics and immunizations. We also have same day appointments available. We file all insurances including Medicare and Medicaid. We also have an income based program for uninsured patients.

To apply for the income based program, one of the following proof of income must be presented at the time of check-in:

• Current paycheck stub
• Social Security Statement
• Letter from DHS with the financial services you receive
  



If no proof of income is available, we will accept a notarized letter from the person/place that is assisting you with your financial care. New patients need to arrive 30 minutes prior to the scheduled appointment time. Established patients need to arrive 15-20 minutes prior to the scheduled appointment time.
